Screening for congenital renovascular hypoplasia and renal artery stenosis by acute converting enzyme inhibition.

To evaluate the usefulness of the captopril test for identifying renal artery stenosis (RAS) and renovascular hypoplasia (RAH), we studied 48 hypertensive patients. In 20 hypertensives with screening procedures indicating renovascular disease and in 28 essential hypertensives (EH), the plasma renin activity (PRA) responses to an oral test dose of captopril (50 mg) were studied. A 60-min post-captopril PRA increase of 150% (or 400% if baseline PRA < or = 3 ng/ml/h) was considered as positive. Renal angiography was performed in all cases. Bolus-enhanced renal spiral CT: technique, diagnostic value and drawbacks

Abstract In addition to pre- and postcontrast renal CT scans, early bolus-enhanced spiral scans during demarcation of the corticomedullary junction were acquired in 85 patients. The diagnostic value and drawbacks of the three imaging series in the evaluation of renal disease were assessed. Renal calcifications and calculus disease detected at precontrast scans (18%) were obscured after contrast administration and excretion in most cases. In the detection of renal lesions bolus-enhanced spiral CT and delayed postcontrast scans had an identical diagnostic yield (94%). Quantifying stenosis in renal arteriograms: a fuzzy syntactic analysis.

AbstractThe introduction of fuzzy logic improves a system for the automatic quantification of renal artery lesions seen in digital subtraction angiograms. A two-step approach has been followed. An earlier system based on non-fuzzy syntactic analysis provided a clear symbolic description of the stenotic lesions. Although this system worked correctly, it did not take into account the variability and uncertainty inherent to image processing and to knowledge on the reference diameter. Regional hypothermia of the kidney: surface or transarterial perfusion cooling? A functional study.

Hypothermia reduces ischemic damage if prolonged occlusion of the renal artery is required during an in situ kidney operation. Hypothermia may be achieved by external cooling or by transarterial hypothermic perfusion. External cooling may lower intrarenal temperature heterogeneously. Perfusion techniques via intra-arterial catheters introduced percutaneously are associated with minimal technical difficulties. The impact of in-situ balloon occlusion of the renal artery and hypothermic perfusion on renal blood flow

Unilateral renal blood flow was evaluated in-situ in 13 dogs by cineangiodensitometry and microsphere distribution studies before and after intermittent balloon occlusion with and without hypothermic perfusion of one kidney. The contralateral kidney served as control. No significant difference in renal blood flow and vascular resistance was noted before, and 5, 30 and 60 minutes after unilateral intra-arterial manipulation. Vascular etiologies are of particular importance as early surgical or minimally invasive intervention can, in some cases, salvage the graft.
